    Urban bicycles renting systems: Modelling and optimization using nature-inspired search methods

    Urban Bicycles Renting Systems (UBRS) are becoming a common and useful component in growing modern cities. For an efficient management and support, the UBRS infrastructure requires the optimation of vehicle routes connecting several bicycle base stations and storage centers. In this study, we model this real-world optimization problem as a capacitated Vehicle Routing Problem (VRP) with multiple depots and the simultaneous need for pickup and delivery at each base station location. Based on the VRP model specification, two nature-inspired computational techniques, evolutionary algorithms and ant colony systems, are presented and their performance in tackling the UBRS problem is investigated. In the evolutionary approach, individuals are encoded as permutations of base stations and then translated to a set of routes subject to the constraints related to vehicle capacity and node demands. In the ant-based approach, ants build complete solutions formed of several subtours servicing a subset of base stations using a single vehicle based on both apriori (the attractiveness of a move based on the known distance or other factors) and aposteriori (pheromone levels accumulated on visited edges) knowledge. Both algorithms are engaged for the UBRS problem using real data from the cities of Barcelona and Valencia. Computational experiments for several scenarios support a good performance of both population-based search methods. Comparative results indicate that better solutions are obtained on the average by the ant colony system approach for both considered cities

    A Hybrid System for Dental Milling Parameters Optimisation

    This study presents a novel hybrid intelligent system which focuses on the optimisation of machine parameters for dental milling purposes based on the following phases. Firstly, an unsupervised neural model extracts the internal structure of a data set describing the model and also the relevant features of the data set which represents the system. Secondly, the dynamic system performance of different variables is specifically modelled using a supervised neural model and identification techniques from relevant features of the data set. This model constitutes the goal function of the production process. Finally, a genetic algorithm is used to optimise the machine parameters from a non parametric fitness function. The reliability of the proposed novel hybrid system is validated with a real industrial use case, based on the optimisation of a high-precision machining centre with five axes for dental milling purposes

    Primary and Secondary Immunodeficiency Diseases in Oncohaematology: Warning Signs, Diagnosis, and Management

    Background: Immunodeficiencies (ID), in particular primary immunodeficiencies (PID), are often associated with haematological manifestations, such as peripheral cytopenias or lymphoproliferative syndromes. Early diagnosis and management have significant prognostic implications. Secondary immunodeficiencies (SID) may also be induced by oncohaematological diseases and their treatments. Haematologists and oncologists must therefore be aware of the association between blood disorders and cancer and ID, and be prepared to offer their patients appropriate treatment without delay. Our aim was to define the warning signs of primary and secondary IDs in paediatric and adult patients with oncohaematological manifestations.Methods: A multidisciplinary group of six experts (2 haematologists, 2 immunologists, and 2 paediatricians specializing in ID) conducted a literature review and prepared a document based on agreements reached an in-person meeting. An external group of 44 IDs specialists from all over Spain assessed the document and were consulted regarding their level of agreement.Results: This document identifies the haematological and extra-haematological diseases that should prompt a suspicion of PIDs in adults and children, in both primary care and haematology and oncology departments. Cytopenia and certain lymphoproliferative disorders are key diagnostic pointers. The diagnosis must be based on a detailed clinical history, physical exploration, complete blood count and standard laboratory tests. The immunological and haematological tests included in the diagnostic process will depend on the care level. Patients who are candidates for immunoglobulin replacement therapy must be carefully selected, and treatment should be offered as soon as possible to avoid the development of complications. Finally, this document recommends procedures for monitoring these patients.Conclusions: This document combines scientific evidence with the opinion of a broad panel of experts, and emphasizes the importance of an early diagnosis and treatment to avoid complications. The resulting document is a useful tool for primary care physicians and specialists who see both adult and paediatric patients with oncohaematological diseases

    Non-motor symptoms burden, mood, and gait problems are the most significant factors contributing to a poor quality of life in non-demented Parkinson's disease patients: Results from the COPPADIS Study Cohort

    [Objective] To identify factors related to a poor health-related and global quality of life (QoL) in a cohort of non-demented Parkinson's disease (PD) patients and compare to a control group.[Methods] The data correspond to the baseline evaluation of the COPPADIS-2015 Study, an observational, 5-year follow-up, multicenter, evaluation study. Three instruments were used to assess QoL: (1) the 39-item Parkinson's disease Questionnaire (PDQ-39), (2) a subjective rating of global QoL (PQ-10), and (3) the EUROHIS-QOL 8-item index (EUROHIS-QOL8). Multiple linear regression methods were used to evaluate the direct impact of different variables on these QoL measures.[Results] QoL was worse in PD patients (n = 692; 62.6 ± 8.9 years old, 60.3% males) than controls (n = 206; 61 ± 8.3 years old, 49.5% males): PDQ-39, 17.1 ± 13.5 vs 4.4 ± 6.3 (p < 0.0001); PQ-10, 7.3 ± 1.6 vs 8.1 ± 1.2 (p < 0.0001); EUROHIS-QOL8, 3.8 ± 0.6 vs 4.2 ± 0.5 (p < 0.0001). A high correlation was observed between PDQ-39 and Non-Motor Symptoms Scale (NMSS) (r = 0.72; p < 0.0001), and PDQ-39 and Beck Depression Inventory-II (BDI-II) (r = 0.65; p < 0.0001). For health-related QoL (PDQ-39), non-motor symptoms burden (NMSS), mood (BDI-II), and gait problems (Freezing Of Gait Questionnaire [FOGQ]) provided the highest contribution to the model (β = 0.32, 0.28, and 0.27, respectively; p < 0.0001); whereas mood and gait problems contributed the most to global QoL (PQ-10, β = -0.46 and −0.21, respectively; EUROHIS-QOL8, β = -0.44 and −0.23, respectively).[Conclusions] QoL is worse in PD patients than in controls. Mood, non-motor symptoms burden, and gait problems seem to be the most relevant factors affecting health-related and global perceived QoL in non-demented PD patients.Peer reviewe
